Heartland serves residents of Nebraska, Iowa and South Dakota. We accept most major insurances and also have three sliding scales for those who do not have health insurance. Payment plans can be arranged on a case-by-case basis. See "Orientation Paperwork" for more details - it is advised that you read through the Orientation Paperwork prior to coming to Heartland. Below are our private pay rates.
Good Faith Estimate as part of "No Surprises Act" effective 01/01/2022:
You may request a "Good Faith Estimate" about out-of-network care to show the cost of items and services that are reasonably expected for your health care needs for an item or service, a diagnosis, and a reason for therapy. More details, including timing of requests/information, can be find in the "No Surprises Act" page of this website.
Co-payments:
All applicable co-payments, deductibles, or any other out-of-pocket expense are expected to be paid at the time of appointment. The co-payment is the patient's responsiblity and payments are expected at the time of appointment unless the patient's insurance coverage requires another arrangement.
Method of Payment:
Payment is accepted by cash, credit card, check, or online via Heartland's In-Sync secure portal - see front desk staff for portal payment access.
